Forms Not Working

Q:  Why is the pdf form in the Forms Warehouse not working?

A:  First, you need to download the form to your PC or laptop or other device.  Trying to fill it in directly online will not enable all the form functions to work.  Second, make sure you have the latest version of Adobe Reader.  The free version is available at https://www.adobe.com/acrobat/pdf-reader.html
